RADICULAR BACK PAIN CARE
Radicular low back pain – the
back pain that radiates down the leg – is often the most irritating.
Researchers noted that chiropractic spinal manipulative therapy
and benzodiazepine drugs were frequently prescribed.
They questioned how spinal manipulation affected
the use of the benzodiazepine drugs in patients aged 18-49 years of age.
The risk of receiving a prescription for a benzodiazepine were
significantly less for those receiving
chiropractic spinal manipulation. The odds did rise a
bit as the back pain stayed: 3 months .56, 6 months
.61, and 12 months .67. The researchers also questioned whether
a patient’s preference to avoid prescription medications affected
this. Their sensitivity analysis didn’t have anything to do with
the risk. The study concluded that
chiropractic spinal manipulation care significantly decreased the
odds of a prescription for and affected the use of
benzodiazepine for radicular low back pain. The patients in the study who started
with the chiropractic manipulation path were more likely to receive
published guideline-directed care for benzodiazepine prescription. (1) This is

OPIOID PRESCRIPTION FOR BACK PAIN
Back pain sufferers with acute onset, preliminary
episode of back pain often go to the emergency room for help.
Opioid prescribing in this setting was reported
as not always following clinical guidelines which recommended
limited use. Researchers back in 2016 reported that such early opioid prescribing for uncomplicated acute low
back pain built up the long-term use of opioids as
well as the cost of care part from the fact that
there are a variety of adverse outcomes associated

The American College of Physicians published recommendations
for care of low back pain in 2009. (3) Spinal manipulation was a recommended
first-line treatment for low back pain in adults. 97%-98% of spinal manipulation providers
were chiropractors. (4) A 2021 follow-up report on whether these recommendations
were followed revealed some eye-opening facts. A
Gallup poll of 5377 random people gave rise to 545 low back
pain sufferers who went to a medical doctor in
the past year. They were asked what form of care
their medical doctor recommended and if they followed that care. 96% said they received a recommendation
of one or more pain treatments. 81% said their medical doctor recommended
drug and non-drug therapies. 76% - NSAIDs and acetaminophen. 79% - self-care
treatments. 37% - massage, acupuncture, or spinal manipulation. 60% - physical
therapy. Nearly 66% said prescriptions (benzodiazepine, opioids,
gabapentin, Neurontin, cortisone injections) were advised.
Follow through on the recommendations by patients ranged from 68%
for acupuncture to 94% for NSAIDs. (5) Apple Country Chiropractic provides
